Service Call Request Form

THIS SECTION MUST BE COMPLETED, FAILURE TO COMPLETE THIS SECTION OR AGREE TO THE TERMS WILL RESULT IN THE SERVICE CALL BEING REJECTED.
We understand that we will be liable for all costs of the repair if the fault is diagnosed as not covered under the terms & conditions of the manufacturers warranty.
Non -Warranty and Charging:
Warranty work will only be undertaken on completion of the required paperwork
You will not be notified that a service call is chargeable before the repairs are completed/invoiced
Service calls completed and are found not to be covered by the manufacturers warranty will be chargeable
Non- warranty charges will vary from one manufacturer to another
By placing the warranty service call the originator confirms acceptance of the terms and conditions of the warranty and understand that payment will be required under the normal trading terms agreed.
We hereby authorise PKL Group (UK) Ltd to invoice our account for any and all repairs completed if the warranty call out is not covered under the terms of the manufacturers warranty. We understand that this payment will be taken under our direct debit agreement.
